[ShaderGraph] Inputs/outputs are duplicated when converting nodes with 2+ outputs from single slot to subgraph
Repro:
1. Create a new ShaderGraph
2. Add an Add node
3. Pipe the output of the Add node to 2+ input slots on the Master Node
4. Convert the Add node to a SubGraph
Result: multiple subgraph output ports are created for the single Add node output slot
