D3D12: device is lost and editor crashes, when a 3D or Array RenderTexture is created with enableRandomWrite and mipmaps
Steps to reproduce:
1. Download and open the attached project
2. Make sure D3D12 is set as the graphics API
3. Open Scene "Sample Scene"
4. Enter Playmode
Result: crash
Reproduced on 2018.1.a01, 2018.1.9f2, 2018.2.17f1, 2018.2.0b11, 2019.1.0a10
API used is not available in 2017.4

Resolution Note (2019.2.X):
this is a missing feature instead of a bug in the code. we would track this issue as a feature for D3D12 and we will get this feature implemented in the near future releases of Unity.
