Standalone Controller Connection Failure
Unplugging the controller from a running build, or starting a build with a controller disconnected prevents you from plugging it back again and using it until you restart the Player.

It seems like the bug still exists in 4.3.4
says fixed in 4.3.8
but in 4.5 it is still there
